A team from the Intermediate Care service has won the first Quality Improvement in action award for NHS Ayrshire & Arran.
The Integrated Care Team (ICT), led by Evelyn Boyle, Integrated Care Manager, was nominated for their work with the medical receiving ward (Station 7) in University Hospital Ayr in increasing the number of timely discharges from the ward.
Andrew Moore, Assistant Nurse Director and lead for Quality Improvement and Governance, explained: “This project highlighted the positive impact of the Integrated Care Team working closely with ward staff ward to improve discharge planning for patients.
“By putting the patient at the centre of discharge planning, patients have been supported to be at home, where possible, with ICT support and home care.”
The Quality Improvement in Action award has been introduced to recognise individuals, teams or services who have delivered a quality improvement initiative, which has demonstrated a sustainable improvement.
Each improvement must demonstrate care that is safe, effective, person-centred and innovative, while demonstrating learning.
The award was presented at the recent quality improvement event, where delegates heard about the work going on across NHS Ayrshire & Arran to improve services for patients.
John Burns, Chief Executive, presented the first award. He added: “It was wonderful to come to this event and hear about the fantastic work going on across the organisation. From empowering patients to self manage, to improving access for patients from care home staff, I was encouraged to see how, by introducing small changes, our staff are making a big difference to patient care.
“On behalf of the organisation, I would like to congratulate Evelyn and her team for winning the award. They have truly demonstrated care that is safe, effective, and more importantly, with the patient at the heart of their care.”
The Quality Improvement in Action award will be awarded four times a year.
